Transitions of Care CPG - AMDA (American Medical Directors Association) has recently published a clinical practice guideline focused on the transition of care in an out of long-term care. This includes AMDA’s universal transfer form and a variety of other resources. AMDA has made a deliberate decision to make this available free of charge for public access.
